What an NPI actually is
A National Provider Identifier is a ten-digit number CMS assigns to a healthcare provider once and never reassigns. It appears on every claim, every prescription and every referral, and it is the only identifier that follows a provider across employers, states and name changes.
The last digit is a check digit, so a mistyped NPI is usually detectable without looking it up at all. This site validates it before it asks the registry.
- Whether the identifier is active or has been deactivated
- The provider's primary taxonomy, and every other one they list
- Practice location and mailing address, which are often different
- State licence numbers attached to each taxonomy
- When the number was issued and when the record was last touched
Two entity types, and the difference matters
NPPES splits providers into individuals and organisations. Of 9,322,332 active records, 7,372,909 are people and 1,949,423 are organisations: hospitals, group practices, pharmacies, suppliers. A group and the clinician inside it both have their own NPI, which is why searching a practice name and a surname return different things.
Deactivated numbers stay in the registry
CMS never deletes. When a provider retires or dies, the record keeps its deactivation date and stays published forever. 349,556 of the 9,671,888 records in the current file are deactivated identifiers, so roughly one in 28 lookups is of a number that no longer works. Every count on this site excludes them, and every record page says which it is.

What this isn't
It isn't a credentialing source and it isn't proof anyone is licensed. NPPES records the licence number a provider supplied at registration; it does not check whether that licence is current, and it carries no disciplinary history. For anything that affects payment or patient care, verify with the state licensing board and with the CMS registry directly.
